How This Works

Yearly cost is the sum of four lines: medication (monthly, times twelve), supplies (monthly, times twelve), provider (per visit times visits, or a monthly subscription times twelve), and labs (per panel times panels). Each line has a low and a high so the output is a range, not a false point estimate.

Choosing a delivery method or a provider model loads a default range into the fields; the defaults are editorial estimates of common US cash prices as of 2026-10 and are meant to be overwritten with real quotes. For an online subscription that bundles medication or labs, tick the matching box and that line drops to zero.

The first-year default uses 4 lab panels and the maintenance default 2, following the monitoring cadence in the Endocrine Society guideline.

Where do the default prices come from?
They are editorial estimates of common US cash prices as of 2026-10, kept deliberately wide. Every field is editable. The most useful way to run the tool is to replace the defaults with the quotes you have actually received.
Why is the range so wide?
Because the medication is usually the cheapest part. The spread comes from the provider model (online subscription, in-person insurance, in-person cash), the delivery method (generic injections versus brand oral capsules), and how many lab panels are drawn in the first year.
Does insurance cover TRT?
Often, for generic injectable testosterone with a documented diagnosis, and less often for brand gels, oral capsules or pellets. Prior authorization requirements are common. The insurance entry in the cost hub goes through what plans typically ask for.
Why does the first year cost more?
Guidelines call for labs at baseline, then at roughly three to six months, then annually once stable, so the first year carries more panels and more visits than a maintenance year.
